Im Mittelalter folgte dann die Seerepublik von Pisa um das Eisen und den Granit abzubauen: Der größte Teil der Säulen, die die Piazza dei Miracoli bereichern, stammen von fähigen Bildhauern aus San Piero. Aus der pisanischen Epoche stammen zahlreiche Zeitzeugen: Die romanische Kirche und der Turm von San Giovanni in Campo, welche aus einer enormen Masse an Granit erbaut wurden, und vor allen Dingen die „Festung“ von Marciana und das Bollwerk von Volterraio, Wachposten für die Berge und das Meer. 1548 folgten die Medici: Cosimo I. realisierte das Befestigungswerk von Portoferraio, ein wahres militärisches Schmuckstück. Die Harmonie des architektonischen Werkes zwischen Land und Meer war so perfekt, dass die “neue“ Stadt „Cosmopoli – Wiege der Zivilisation und Kultur sowie Beispiel für Ausgewogenheit und Rationalität“, genannt wurde.
